
Community Health Grants support health and wellness projects which improve community health through channels such as prevention, health maintenance, screening, and education. The purpose of this grant program is to enhance the health status of citizens served by Central Georgia Health System.
APPLICATION EVALUATION
Applicants may address any factors underlying a healthy community. Grants are particularly encouraged which:
· exhibit partnering and collaboration between and among groups such as but not limited to: governmental agencies, educational institutions, arts organizations, health care providers, criminal justice agencies, chambers of commerce, insurance providers, civic organizations, employers, the media, etc.
· address needs currently unmet
· are cost effective
· demonstrate positive measurable outcomes
· are new or advance existing programs to a higher level of innovation
· emphasize prevention and education
· are time specific; grants are not intended to sustain long-term operation
ELIGIBILITY CRITERIA
Grant requests which assist one or a limited number of specific individuals and/or researched based proposals are not considered. Capital requests, as well as program requests are eligible for consideration. An applicant organization must provide a copy of a letter from the Internal Revenue Service stating that it is tax-exempt under section 501 (c) (3) of the Internal Revenue Code and that it is not a private foundation as defined in section 509 (a) of that code. A proposing organization must have a commitment to improve and enhance community health as evidenced by its mission and purpose. Additional consideration may be given to an organization working in concert or partnership with a unit of Central Georgia Health System. The lack of such affiliation will not, however, disqualify an organization from receiving funding. If Medical Center of Central Georgia administrators are required to review your application, please allow two weeks for their review.
APPLICATION PROCEDURE
Organizations considering application for a Medcen Community Health Grant are required to provide a one page letter of intent (LOI). If the committee and Board determine that the proposed project meets our funding objectives, we will notify the organization and request additional information. The LOI should include the name and a brief description of the project, including its purpose, estimated cost, and any community partners in the proposed project. In order to give all organizations equal consideration, we ask that Board and committee members not be individually solicited. (DO NOT INCLUDE APPLICATION MATERIALS WITH LOI.)
SCORING
The following factors are weighted equally in the evaluation process:
· The problem being addressed is clearly defined, is compelling and is worthy of funding.
· The primary aims and objectives of the proposal are stated clearly and of merit.
· The proposal addresses unmet need(s), is new or advances an existing program to a higher level of innovation.
· Collaborative efforts with other organizations are documented and significant in addressing the problem.
· Expected benefits and outcomes are defined, measurable and will have a meaningful impact on community health.
· The budget is rational, reasonable and appropriate to achieving the desired aims and objectives.
SELECTION PROCESS
The Board of Medcen Community Health Foundation has final approval of all grant requests. A Community Grants Selection Committee, consisting of Foundation Trustees, may review proposals and make recommendations to the Full Board. Assistance in assessing proposals may be sought from outside healthcare professionals and/or community representatives.
Following the review of eligible applications, the Selection Committee and/or Full Board reserve the right to request additional information or to meet with applicants. No individual staff member or Trustee can commit the Foundation; therefore, solicitation of individuals is discouraged.
NOTIFICATION
Applicant organizations will be informed in writing of Trustee decisions. Please do not contact the office directly regarding approval decisions.
REPORTING REQUIREMENTS
Recipient organizations shall generally report one year following receipt of the grant unless otherwise stipulated at the time of approval. Past reporting and performance may be considered when making current awards. All final reports must certify that all terms of the grant have been fully met.
Recipient organizations must agree to cooperate with any public announcements, and/or programs arranged or supported by the Foundation to publicize the benefits of the grant to the community.
Any publications or products derived from funded projects must designate the support received from the Foundation.
